The Private Practice Prescription
There was a time when the “MD” designation was instantly and almost universally synonymous with financial success. However, in the last decade or so, it has become increasingly difficult for physicians in private practice to remain profitable. While there are always opportunities within large hospitals or attached to healthcare network facilities, many doctors would rather be able to provide a smaller clientele with a higher quality of one-on-one patient care. Unfortunately, insurance reimbursement is generally so low that these private practitioners are often losing money by accepting major medical plans. Additionally, the cost of malpractice insurance continues to rise and drug, supply and equipment expenses are simply off the charts. Regrettably there are no easy solutions; however, there are a few very basic ways to cut office expenditures, such as reduction of non-essential staff and purchasing of refurbished medical equipment.
“Equipment” of almost any description can be exorbitant; particularly so for the private practice doctor who bears the responsibility of equipping and outfitting an entire office. A single exam table can cost thousands of dollars, likewise, waiting room furnishings, exam instruments, computers and the like, all carry hefty price tags. Whether a physician is just entering into private practice or is attempting to keep an existing practice afloat, refurbished medical equipment can save upwards of 50-percent off of retail. Also, used furniture, furnishings, fixtures, computer equipment and telephone systems can save you a bundle.
Another area of potentially significant cost savings is in staffing. Many of the day to day functions of a busy practice could actually be outsourced. Dictation services, digitizing of patient records, accounting and payroll (for example) can all be handled by independent contractors for a fraction of the cost of a fulltime staff member. No one wants to have to lose valued employees, but when it becomes an issue of sink or swim, it’s best to drop excess weight wherever possible.
Finally, there is almost always room for improvement when it comes to property rental and service providers (phone, internet, vendors, etc.). Review your expenses in these areas annually to determine if it’s time to consider a switch.
Private practices are failing at an alarming rate across the US, but with careful budgeting, and cost-cutting wherever possible, you can keep your practice alive and healthy.

Different Types of Steam Boiler
A steam boiler or steam generator has a number of possible uses and have been popular since the turn of the 20th century. A boiler generates hot steam to power trains, ships, road vehicles and other types of engines and machinery. These boilers also can warm trains and heat water in the locomotives. Depending on your needs and comfort level, you can purchase an electric boiler or traditional model.
The traditional model requires the use of gas, oil or similar fuel. Thus, there is a risk of leaks and potentially life-threatening explosions.

An electric boiler requires less maintenance though, usually needing only a weekly washing and a check about every three months for water leaks. Water leaks are not necessarily dangerous but can reduce the machine’s effectiveness. When maintained properly, an electric boiler converts electricity into thermal energy at nearly 100 percent effectiveness.
No matter what type of boiler you buy, always make sure it is up to ASME code for everyone’s safety. Using a reputable company that specializes in such equipment is recommended so you know the true specifications of your energy-converting machinery.

There is no experience quite like seeing lions in the wild. When you see them out in their native habitat, you can truly see that they are the kings of all they survey. Steam Power – A Deadly History
Around 2:00 AM on April 27, 1865 an enormous explosion lit up the Mississippi River not too far north of Memphis, Tennessee. More than half (actual numbers vary) of the 2,400 passengers and crewmen of the ill-fated steamer “Sultana”, died as a result of an inadequately repaired boiler.
In the mid nineteenth century, such unfortunate mishaps were considered, almost expected. Boilers exploded, often multiple times in a week. In fact it has been estimated that approximately 1,000 explosions occurred between 1880 and 1886, totaling over $3-million in damages. Steam power was, by its very nature dangerous and unfortunately, lives were sometimes lost. In this safety-conscious day and age it’s difficult to understand an “acceptable loss” scenario such as this. We must, however, remember the vital role steam power played at the time. Steam locomotives and ships were capable of transporting large numbers of people, livestock and substantial payloads, over great distances, in a way nothing else could. Steam power was quite simply irreplaceable. It is true that most operators in the mid to late 1800’s were largely ignorant of the steam machine. As with any new technology, there is always a learning curve; the only way these individuals had of understanding how steam power worked, how to maintain and repair such equipment and how to anticipate failures; was with time, experience and observation.
As a result of these most notorious accidents, the Sultana in particular; people sat up and began to pay attention to the inherent dangers of steam power. Ultimately a handful of enterprising and scientifically minded men decided that under the right conditions, operators could be induced to perform (or submit to) safety inspections, allowing for potential problems to be diagnosed and corrected before an explosion occurred. This was the birth of the [steam boiler] inspection and insurance business.
Fast forward to present time. Steam power is infinitely safer than it was 150 years ago. Equipment is now manufactured under the strictest of regulations and guidelines and operators are vastly more knowledgeable about the equipment under their care. It is indeed unfortunate that lives had to be lost but it is a true testament to the value of steam power that we still rely on it today.

Not every one of us knows much about computerized physician order entry systems or CPOE. Usually we go the hospital or to see our physician only at the time of any problem and we don’t try to find out anything about the backend working of the hospital. Everything seems to be pretty simple in a hospital whereas actually it isn’t that simple. It is pretty hard to run a hospital successfully; in fact managing a hospital is one of the most difficult things to do because a small mistake can take someone’s life. It’s a very responsible and hectic job to run a hospital.
